If I have agreed to write a recommendation for you, please provide me with a copy of your transcript a copy of your personal statement (if applicable) all required forms (with as much information filled out as possible) the letter of recommendation waver form from your senior tutor stamped and addressed envelopes

Also please email me brief answers to as many of the following questions as you can as soon as possible: What is your year and concentration? For what are you applying? Please send me a list of the programs and due dates to which you are applying. How would you describe yourself? What are some of your academic accomplishments? What are some of your nonacademic accomplishments? What are your long terms goals and how would a successful application help you achieve them? Anything else that you think I should know for the purposes of writing your recommendation letter.

Last but not least, please be sure to send me a reminder two weeks before your recommendation letter is due. I'm usually prompt but sometimes I do get swamped.
